Overview

This presentation explores the growing social, emotional, and practical challenges faced by individuals living with terminal illness who lack adequate caregiving support, housing, or financial resources at the end of life. Through real-life stories and the development of Orange County’s first nonprofit residential end-of-life care home, attendees will gain insight into the “social model” of hospice care and the importance of community-based support systems that complement traditional medical hospice services. Participants will also explore the role faith community nurses play in recognizing unmet needs, supporting caregivers, and connecting patients and families with compassionate community resources that improve quality of life and dignity at the end of life.


Objectives

At the conclusion of this activity, participants will be able to:


  1. Identify common social and caregiving barriers that impact quality end-of-life care for patients and families.
  2. Describe how community-based supportive care models can improve dignity, comfort, and emotional well-being for vulnerable hospice patients.
  3. Discuss the role of faith community nurses in recognizing psychosocial needs, supporting caregivers, and connecting patients with appropriate end-of-life resources and services.

Registration

Hoag Faith Community Nursing monthly meeting presents The Social Model of Hospice Care: Bridging the Gap in End-of-Life Support.  This course is FREE and open to all Hoag FCNs, as well as all those interested in the topic. Lunch will be provided. 1 CEU credit for this presentation. 


Accreditation

Hoag Memorial Hospital Presbyterian is accredited by California Board of Registered Nurses to provide continuing education for nurses. This Live Activity is designated for the maximum of 1.0 contact hours. (BRN Provider: 17163)
